Duties: As a Care Coordinator, Employee shall perform the following duties and obligations as part of Employee’s employment:

  1. Monitoring clients in treatment to support recovery.
  2. Maintaining client records in client management system.
  3. Preparing written reports as needed.
  4. Maintaining positive relationships with referral sources and outside providers.
  5. Participating in implementing client’s treatment.
  6. Maintaining communication with client’s, family members and counselors.
  7. Responsible for urine testing/breathalyzers and recording of tests and results.
  8. Safety checks on the grounds of the facility.
  9. Maintaining the sign-in/sign-out sheets.
  10. Maintaining the communication book.
  11. Inspecting and making sure that all work details assigned to the clients are completed.
  12. Responsible for the security of our program.
  13. Monitoring and documenting client’s behavior.
  14. Responsible for reporting and investigating (as requested) all incidents concerning client  immediately to Foundations.
  15. Promptly reports to the Case Manager any problems concerning the van, maintenance, supplies or any household problems.
  16. Interfacing with clients, referring their problems to the proper person resulting in the clients getting the assistance they need.
  17. Overseeing periodic special work details.
  18. Serving as a role model to clients.
  19. Performing crisis intervention when necessary.
  20. Arranging transportation of clients to meetings, hospitals, etc. and if need be, may be asked to drive.
  21. Performing tasks delegated by Employee’s immediate supervisor.
  22. Deescalate any conflicts or potentially violent situations with clients.
  23. Comply with Foundations’ policies and procedures.
